Instance creation failing ( Block Device Mapping)

Hi All.

I seek to install Oracle Openstack Kilo for HA.

I done all most all configuration as per the document says. But I am not able to spin of the instances from Dash_Board. I see below error. 

Error: Failed to perform requested operation on instance "test06", the instance has an error status: Please try again later [Error: Build of instance e48ca233-3553-4a0a-82de-fb61f80fda8f aborted: Failure prepping block device.].

If anyone can help it would more help to me.

How are you trying to launch a vm. Is it boot from image or boot from volume?

Is volume created successfully?

Launching from Boot from image. Yes value is creating perfectly.

You should also check glance and cinder logs for identifying exact problem, this issue seems more related to cinder or glance side.

Yes as per the suggestion I've crossed checked from Cinder and Glance services logs. From those 2 services no log error.

For our Information, the logs are below.

More info would be helpful, Any errors in nova-conductor.log ?
