Windows instance fails to boot, virtual disk changes

Openstack Kilo Ubuntu 14.04

I can no longer successfully deploy a Windows guest (2012R2) from a Windows Image I create a few weeks ago. The VM just crashes with a blue screen.

The reason why the vm crashes is because the virtual disk type changes from VirtIO to SCSI when deployed. I checked my original template that I built using virt-manager and the disk is VirtIO. I'm positive that it worked initially but now it keeps deploying the VM with a SCSI disk.

If I manually shutdown the VM using virt-manager and change the disk type the VM will boot fine but as soon as the VM boots, openstack shuts it down and switches the disk type.

Any ideas?

After running apt-get update/upgrade and dist-upgrade, the problem seems to have resolved itself.
