How to interpret nova instance vm_state = "error"

I see many such entries in the nova.instances sql table:

My interpretation of what has happened when vm_state = "error"
1. launched_at field is null: error occurred while attempting to launch
2. launch_at is populated, but terminated_at is null: error occurred while attempting to terminate, although VM instance had been created and launched successfully.
3. terminated_at is populated, but deleted_at is null: error occurred while attempting to delete, although VM instance had been created and launched successfully.
4. launced_at, terminated_at, deleted_at all have valid timestamp entries: What does this mean? VM instance had been launched and deleted successfully? If so, what could have cause the "error" in vm_state?

Is my reasoning for 1 - 3 correct? And how do I interpret 4?
