What should I put into Schedule A inside CCLA?

When I signup Corporate Contributor License Agreement (CCLA) the form asks me to fill up dedicated employee names in Schedule A, but what exactly is the name? Is it their name as specified in Openstack ID? is it their name in Launchpad?

The Schedule A should contain the legal name of the people authorized by the corporation to contribute on its behalf. Since the Schedule A has no other practical purpose than the legal archive, adding the Launchpad ID or other nicknames wouldn't have any advantage.
