Are you by any chance using a volume for the instance? If so, The compute service calculates the disk space based on the flavor size.

The compute service looks at the available disk space in /var/lib/nova/ if it does not match what is there based on the flavor size calculations it gives that warning. You can see what is expected using the $ nova host-describe command.

If you are using ephemeral disks it is possible to see this effect, but it is because you are not using all of the disk space allocated to your flavor.

I've also ran into this issue.. If you're account is limited to 10 volumes, and you attempt to create a new instance, and this new instance uses a new volume, I will see the error that the OP is getting

I'm not sure exactly what could be causing this, but it appears someone else had luck deleting errored instances, and rebooting controller node and launching again. Below is a question that appears to be hitting on the same topic as yours, hope it helps.
