When does nova go in fail state

Hello people,

For my company i need to know what the criteria are for nova to go in fail state. By this i mean when does nova seizes to stop functioning and needs to be evacuated. As an example: it has 5 bad sectors and now it needs to evacuate or 1 disk failed in the RAID setup (which it's a pretty obvious reason for evac). Is there documentation on this where i can read the criteria on this.

For clarification: there is no issue, this is an information request topic.
