What distribution are you running? What version of packstack are you running (rpm -q openstack-packstack)? Which repositories are defined on your system (yum repolist -v)?

can you add the log in

You will find full trace in log /var/tmp/packstack/20141021-123917-R0sV6Z/manifests/10.150.68.22_mariadb.pp.log

it happened to me recently ... if it isn't your first run of packstack the password for mysql will be wrong. try log to your mysql server: mysql -u <usernae> -p than enter the pass and see if you can connect if so go to your home folder and edit the answer file you see there it should be something like packstac-answer-file-XXXXX

edit it whit the password and try runnong packstacl again packstack --answer-fileXXX
