Cannot attach volume to a particular instance

I have created a 2TB volume in our OpenStack frontend and wanted to attach the volume to a particular VM. When attach request is made for the particular instance, the request times out with a mount failed result.

I reached out to our openstack administrator and he replied that cinder, nova, qemu & libvirt doesn't log anything. He tried to attach device via command-line and got the response:

ERROR: The server has either erred or is incapable of performing the requested operation. (HTTP 500) (Request-ID: req-779d574bdb51-4738-8d55-500c3deb74b3)

Problem is isolated into one VM and probably something strange is going on in the VM's OS. However, the VM doesn't log anything when I try to attach a volume. Funny thing is I can attach the volume to other instances and the problematic instance has already an attached volume.

add --debug to the command so you can get more output, it may have useful information on why it is failing.
