How to declare the fieldtype of a dropdown box to be added in the login page of horizon ?

Hi,

I have a code like this where I need to edit for bringing a extra label in my existing login page of horizon. I need to bring an extra label as a dropdown box in the existing login page. Please help me with the changes I need to do with the fieldtype for a dropdown box.

class AuthenticationForm(forms.Form):
    """
    Base class for authenticating users. Extend this to get a form that accepts

    """
    username = forms.CharField(max_length=254)
    password = forms.CharField(label="Enter Password", widget=forms.PasswordInput)
