Rebooting forever and Failed to connect to server

Somehow today my VM has some issues and I decided to reboot it. But since then the dashboard shows "Reboot Started Hard" and it's there forever. I cannot access to the console either and it says "Failed to connect to server (code: 1006)"

What should I do to get my VM back?

I think "Did not find any data source" is the key to your problem. It probably means that cloud-init doesn't get anything from the metadata service.

Check the health of the Neutron metadata service. Try other instances from other images.

I am using the cloud service from a service provider, so what should I do? Would it be possible for the service provider solve the issue? Or I have to just forgo it and just start a new instance from scratch? Thanks.

You have to solve the problem in your i stance, where the cloud provider doesn't have "jurisdiction". They might have hints and tips, though.

To check the metadata service's health, however, you need to talk to the provider.

Thanks, but I cannot even connect to the console, does it means I would not be able to solve it and must forgo it?

If you can't connect to the console, the cloud has a configuration problem that only the provider can solve.
