Everything seems to be okay but I cannot seem to be able to create an instance. At first, I had issues with Placement service (earlier it would go into BUILD state indefinitely) which has been remedied but now it errors out. In the Nova logs, however, I don't see any errors or warnings. Would be nice if the exception is thrown or some message is provided when an ERROR condition occurs.

Any suggestion on how to troubleshoot this failure?

Have you checked the nova-compute log on the compute node?

What is the output of openstack server show selfservice-instance?

What happens when you launch another instance?

Can you post error during create instance failed? (On dashboard screen) Please aware on: Disk space for CINDER volume, and does root disk is enough for OS installed)

In order to troubleshoot this, set:

in /etc/nova/nova.conf

[DEFAULT]
(...)
debug = true
(...)

Do this on all controllers, and on the compute node where the instance spawns (if this is a small lab, don't bother and just switch debugging on everywhere. Otherwise, use availability-zones to select a hypervisor:

[stack@undercloud ~]$ nova availability-zone-list

[stack@undercloud ~]$ nova boot ----availability-zone nova:overcloud-compute-0.localdomain (...)

Once you set debug = true, restart all OpenStack nova services on the controllers and on the compute node(s) to apply the configuration change:

systemctl list-units | grep nova | awk '{print $1}' | xargs -I {} systemctl restart {}

Finally, spawn a new instance, wait until it goes into ERROR. Then, grep for the instance's UUID in /var/log/nova/*.log on all controllers and the compute node(s).
