Please verify the following things to diagnose the issue:-

1. Make sure that your device partition type of which you have created the physical volume and volume group for cinder is setup to 'Linux LVM' type(8e) as shown below:-

/dev/sda3 6146048 88066047 40960000 8e Linux LVM

1. Make sure that name of the volume group your created is 'cinder-volumes'. It should look like below:-

[root@controller ~]# vgs VG #PV #LV #SN Attr VSize VFree cinder-volumes 1 1 0 wz--n- 39.06g 34.06g

1. To get more verbose output and detailed error logs please set the value of followings to true in cinder configuration file:-

debug=True

verbose=True

After changing the configuration in cinder.conf, please restart the cinder services and try to perform the operations again.

After checking with above things you should be able to diagnose the issue, else provide the cinder logs.
